艾灸与针刺对局部穴区组织免疫及代谢调节机制的对比研究

<p style="text-align: center;">刘 霞<sup>1</sup>,钟 欢<sup>1</sup>,刘 琼<sup>1</sup>,刘 涛<sup>1</sup>,曹佳男<sup>1</sup>,张 驰<sup>1</sup>,常小荣<sup>1</sup></p><p style="text-align: center;">(1湖南中医药大学针灸推拿学院,长沙410007)</p><p style="text-align: justify;">摘要:目的:1.观察艾灸与针刺对生理状态及慢性萎缩性胃炎(CAG)病理状态下大鼠局部穴区组织免疫细胞、免疫相关因子及代谢的影响,探讨艾灸与针刺起效的共性与特性作用。方法:1.通过MNNG联合多因素复制CAG模型大鼠,以艾灸和针刺作为干预手段,设置正常组、正常+灸组、正常+针组、模型组、模型+灸组、模型+针组,按照各组施加的被试因素进行干预,HE染色观察各组大鼠胃组织病理学特点,甲苯胺蓝染色观察局部穴区组织肥大细胞的含量;2.免疫组化法检测局部穴区组织中神经肽(SP、NPY、CGRP、缓激肽)、组胺受体、炎症因子(IL-1β、IL-6、IL-10、IL-18、TNF-α)的含量;3.利用核磁共振氢谱技术(1H NMR)检测局部穴区组织中代谢物的变化,通过PCA、PLS-DA、OPLS-DA等分析方法对局部穴区组织做代谢模式、差异性代谢物分析。结果:1. 生理及病理状态下,针刺及艾灸“足三里”穴区肥大细胞数量多于模型组,艾灸组显著高于模型组,差异具有统计学意义。2.艾灸对局部“足三里”穴区组织NPY、缓激肽有显著影响(P<0.05;P<0.01),且这种变化在CAG病理状态和生理状态一致。针刺对局部“足三里”穴区组织CGRP、SP有显著影响(P<0.05;P<0.01),且这种变化在CAG病理状态和生理状态一致。在CAG病理状态下“足三里”局部穴区组织处于炎症状态,艾灸作用可增强炎症的程度,针刺能降低炎症状态。3. 艾灸与针刺两种刺激方式对局部“足三里”穴区组织代谢有不同的影响,艾灸影响了局部组织的能量代谢、糖酵解、核酸代谢、牛磺酸代谢、磷酸胆碱代谢。针刺主要影响了局部组织的能量代谢、氨基酸代谢、核酸代谢和糖酵解。在CAG病理状态下局部“足三里”穴区组织代谢发生了紊乱,主要表现在神经递质代谢紊乱和能量代谢紊乱。艾灸干预后主要调节了局部穴区组织紊乱的神经递质,对能量代谢也起到了调节作用;针刺干预后主要调节了局部穴区组织神经递质的代谢紊乱状态。结论:1.艾灸与针刺对局部穴区组织免疫细胞的调节存在差异,艾灸与针刺均能促进局部穴区组织免疫细胞的聚集,艾灸作用强于针刺。2.生理状态下艾灸与针刺均通过上调局部穴区组织神经肽、炎症因子起到生物学效应。病理状态下,艾灸对穴区组织炎症因子表现为上调作用,而针刺对穴区组织炎症因子表现为下调作用。3.生理状态下,艾灸倾向于促进局部组织细胞的凋亡,针刺倾向于神经递质的调控。病理状态下,艾灸对局部穴区组织中紊乱的神经递质及能量代谢有调控作用,而针刺主要调控了局部穴区组织中紊乱的神经递质。</p><p>关键词:艾灸;针刺;代谢组学;穴区组织;差异性研究</p><p style="text-align: center;">Comparative study of regulation mechanism between Moxibustion and Acupuncture on local point immune and metabolic</p><p style="text-align: center;">LIU Xia<sup>1</sup>, ZHONG Huan<sup>1</sup>, LIU Qiong<sup>1</sup>, LIU Tao<sup>1</sup>, CAO Jianan<sup>1</sup>, ZHANG Chi<sup>1</sup>, CHANG Xiaorong1</p><p style="text-align: center;">(1 College of Acu-moxibustion and Massage, Hunan University of Chinese Medicine, Changsha 410007, China)&nbsp;</p><p style="text-align: justify;">Abstract:Objective: 1. To observe the effects of moxibustion and acupuncture on local point area organization immune cells, immune factors and metabolism in physiology and CAG pathological state; 2. To reveal the regulation mechanism differences between Moxibustion and Acupuncture on local points area on physiology and CAG pathological. Methods: 1. MNNG ally multi-factor method replicate CAG model rats, with acupuncture and moxibustion as intervention, set up the normal group, normal+moxibustion group and normal+acupuncture group, model group, model+moxibustion group, model+acupuncture group, according to the subjects factors intervention groups, HE staining to observe each group rats gastric histopathological features; 2. The immunohistochemical method to detect the local points area tissue neuropeptide (SP, NPY, CGRP, slow excitation peptide), histamine receptor, inflammatory factor (IL-1 beta, IL - 6, IL - 10, IL - 18, TNF alpha) content;3. By using 1 H NMR detection of metabolites in local points area organization, through the PCA, PLS - DA, OPLS - DA analysis methods to explore the commonness and differences between Moxibustion and Acupuncture.Results: 1. &quot;ST 36&quot; area number of mast cells in Model + moxibustion group and model+acupuncture group are more than model group, model plus moxibustion group was significantly higher than that of model group, the mean comparison (P&lt;0.05, the difference is statistically significant. Acceptance of moxibustion or normal rats foot after the &quot;ST 36&quot; acupuncture intervention zone tissue mast cells also has increasing trend, moxibustion also has significant difference, P&lt;0.05).2. The way of moxibustion and acupuncture stimulation of local &quot; ST 36&quot; hole area tissue neuropeptide expressions have different influence, moxibustion on local &quot;ST 36&quot; point area organization NPY, slow excitation peptide has significant effect (P&lt;0.05; P&lt;0.01), and the change in the CAG pathological and physiological state. &quot;ST 36&quot; acupuncture on local area organization CGRP and SP has significant effect (P&lt;0.05; P&lt;0.01), and the change in the CAG pathological and physiological state. In the CAG pathological state of &quot;ST 36&quot; local hole area organization is in a state of inflammation, moxibustion can enhance the degree of inflammation, acupuncture can reduce the inflammatory state.3. The way of moxibustion and acupuncture stimulation of local &quot;ST 36&quot; point area tissue metabolism have different effects, moxibustion has affected the local tissue of energy metabolism, glycolysis, nucleic acid metabolism, taurine, choline phosphate metabolism. Acupuncture mainly affects the local tissue of energy metabolism, amino acid metabolism, nucleic acid metabolism and glycolysis. In the CAG pathological state local &quot;ST 36&quot; point area tissue metabolism disorders, mainly displays in the neurotransmitter metabolic disorders and energy metabolism disorders. After moxibustion intervention mainly adjust the local hole area organization disorders of neurotransmitter, on energy metabolism also play a regulatory role; After acupuncture intervention mainly adjust the local hole area organization the neurotransmitter metabolic disorder state. Conclusion:1. Moxibustion and acupuncture have different regulation effects on local points area organization&nbsp; of immune cells, the commonness of moxibustion and acupuncture regulation is promote the local point district organization the accumulation of the immune cells, and moxibustion effect is better than acupuncture, the difference has nothing to do with the body&#39;s disease states.2. The generality of the physiological state of moxibustion and acupuncture in order to promote the local hole area of tissue inflammation state, members of the opposite sex as proinflammatory role way different. The pathological state of moxibustion and acupuncture for adjusting the local points area organization of the generality of the inflammatory state, heterosexual as the moxibustion as proinflammatory response, acupuncture tend to inhibition of inflammation.3. Physiological state of moxibustion and acupuncture for the generality of the metabolic state of active, the opposite sex for the biological effect difference, moxibustion tend to promote the local tissue cells apoptosis, acupuncture tend to the neurotransmitter regulation. The pathological state of moxibustion and acupuncture for the generality of the metabolic state regulation of the opposite sex for the biological effect difference, moxibustion on local points area organizations disorder in the neurotransmitter and energy metabolism has a regulatory role, and acupuncture mainly control the local points area disorders of neurotransmitter in the organizations.</p><p>Key Words: Moxibustion; Acupuncture; Metabonomics; local point organization; Comparative study</p><p><br/></p>
