<p style="text-align: center;">张医超，范　敏，张医阳</p><p style="text-align: center;">(河南省张碧波中医针灸研究院，河南 郑州 450000)</p><p style="text-align: justify;">摘要：骨折之后，需先将骨复位，后通调气血，通经活络。神庭穴属督脉，为督脉、足太阳、足阳明之会。督脉属奇经八脉之一，是人体诸阳经脉之总汇，总督一身之阳，为“阳脉之海”。印堂穴为经外奇穴，位于督脉循行路线上，取之即可调节督脉精气，又可调节肾气肾精，充养髓海，通调十二经脉之气。针刺疏通经络、调补气血，而透刺又有独到疏通经气之效。故印堂透刺神庭可激发人体之阳气，通调十二经脉，共同起到调节肾气肾精、充养髓海、调补气血、镇静止痛之效。</p><p style="text-align: justify;">关键词：骨折；印堂；神庭；透刺</p><p style="text-align: justify;"><br/></p><p style="text-align: justify;">Abstract: After a fracture, one needs to restore the bones at first and regulate qi and blood, clear and activate the channels and collaterals afterwards. DU24 belongs to Governor Vessel, which is the converging point of Gallbladder Meridian of foot greater yang and Stomach meridian of food yang brightness. Governor Vessel is one of the eight extra meridians, which is the confluence of yang meridians and collaterals in human body, governing yang of the entire body, which is also regarded as “the sea of yang”. EX-HN3 is an extra-meridian point which locates on the running route of Governor Vessel. By acupuncturing this point, the essence of Governor Vessel can be adjusted, the qi and essence of kidney can be regulated in order to fill the marrow sea and regulate qi of the twelve meridians. Acupuncture this point can dredging the meridians and collaterals, tonify qi and blood, while penetrating this point has the unique function of dredging channel-qi. Thus, penetrate from EX-HN3 to DU24 can trigger yang qi in the body, adjust the twelve meridians, together both points can regulate kidney qi and essence, fill the marrow sea, tonify qi and blood and relieve pain.&nbsp;</p><p>Key words: Acupuncture and moxibustion, fracture, EX-HN3, DU24, penetrate from EX-HN3 to DU24</p><p><br/></p><p style="text-align: justify;">&nbsp; &nbsp; &nbsp; &nbsp; 患者詹某，女，40岁，2016年12月24日就诊。患者当日骑电动车与车碰撞摔倒后导致足部跖骨骨折，医院DR片显示：左足第2、3跖骨远端骨质连续性中断，断端可见成角移位，跖趾关节对位不佳。医院嘱其尽快手术治疗，患者不愿手术，遂来诊治。患者突然骨折，关节对位不佳，需先将骨复位，后通调气血，通经活络。选穴：印堂、神庭，针法：透刺，手法：捻转补泻之补法。</p><p style="text-align: justify;">&nbsp; &nbsp; &nbsp; &nbsp;患者取仰卧位，常规消毒，选用泉河牌0.3 mm×50 mm不锈钢毫针，用夹持进针法快速刺入印堂穴，针尖向上，平行进针，向神庭方向透刺，边进针边捻转，使用捻转补泻之补法，得气后留针30 min。针一次后患者疼痛明显减轻，可下床走路，治疗11 d后(2017年1月6日)患者DR片显示：左足构成骨骨质未见异常，各关节对位好，关节间隙不窄，骨性关节面光整。骨折已愈合。</p><p style="text-align: justify;">&nbsp; &nbsp; &nbsp; &nbsp;神庭穴属督脉，为督脉、足太阳、足阳明之会。督脉属奇经八脉之一，是人体诸阳经脉之总汇，总督一身之阳，为“阳脉之海”。《素问•骨空论》载：“督脉者，起于少腹……足太阳起于目内眦，上额交巅上，入络脑，还出别下项，循肩膊内，侠脊抵腰中，入循膂络肾[1]。”《针灸大成》曰：“以人之脉络，周流于诸阳之分，譬犹水也，而督脉为之督纲，故名曰海焉[2]”，明确指出督脉统领诸经，对各经病变均有调整作用。印堂穴为经外奇穴，位于督脉循行路线上，取之即可调节督脉精气，又可调节肾气肾精，充养髓海，通调十二经脉之气。针刺疏通经络、调补气血，而透刺又有独到疏通经气之效。故印堂透刺神庭可激发人体之阳气，通调十二经脉，共同起到调节肾气肾精、充养髓海、调补气血、镇静止痛之效。闸门控制学说认为疼痛传导时脊髓部位有可以被调制的胶状质细胞，调节脊髓阶段的神经通路，疼痛可以被抑制。印堂透刺天庭，可调节肾气肾精，肾主骨生髓，针刺通过调节脊髓阶段的神经通路而止痛。目前西医对跖骨骨折的治疗方法一般为手术治疗，且骨折愈合时间为6~12周，平均8.4周，而本案病例针刺一次即疼痛明显减轻，治疗11 d骨折即愈合。而目前尚未查到有采用印堂透刺天庭治疗骨折之病例，现将此法公布，以抛砖引玉。笔者临证还用此穴治疗急性踝关节扭伤、急性腰扭伤呢，采用印堂穴治疗膈肌痉挛，均取得佳效。</p><p>参考文献</p><p>[1]黄帝内经•素问[M].姚春鹏，译注.北京：中华书局，2010:114.</p><p>[2]杨继洲.针灸大成[M].北京：人民卫生出版社，2006:34.</p><p><br/></p><p><br/></p>
