健康人体背部经脉(穴)皮肤微循环灌注量特征及艾灸命门穴对其影响的试验研究

<p style="text-align: center;">郭 扬1,王 舒2,杨华元3</p><p style="text-align: center;">1.天津中医药大学,天津,300193;2.天津中医药大学第一附属医院,天津,300193;3.上海中医药大学,上海,200193</p><p style="text-align: justify;">摘要:目的 通过观察健康人体本态下背部经脉(穴)皮肤微循环灌注量(Microcirculatory Perfusion Unit)及艾灸命门穴后相关经穴皮肤微循环灌注量的变化,为经脉的特异性及灸法的作用原理提供依据。方法 以健康受试者19人为研究对象,运用激光散斑血流视频监测系统(The PeriCam Perfusion Speckle Imager System, Sweden),将背部分为上、中、下三段范围进行检测,分别监测安静状态下、艾灸命门5-30min、灸后5-30 min (每五分钟检测一次)背部经脉(穴)皮肤微循环灌注量,以获取原始数据,然后运用线性统计(医学统计)方法,探寻背部经脉(穴)皮肤微循环灌注量特征。结果 健康人体督脉线皮肤血流量高于膀胱经第一侧线皮肤血流量,左右侧膀胱经脉线皮肤血流量存在差异(p&lt;0.05);艾灸命门可使健康人督脉及膀胱经线皮肤血流量升高,推测经脉具有一定的传导热的功能。结论 督脉与膀胱经经脉线皮肤血流量存在差异,左右侧膀胱经经脉线血流存在差异,经脉线与非经线相比具有特异性;艾灸干预下,相关经脉线皮肤血流量会发生特异性变化,经脉线具有传导热的特异性。</p><p style="text-align: justify;">关键词:督脉;艾灸; 激光散斑;皮肤微循环;灌注量</p><p style="text-align: center;"><strong>The Characteristic Study of Microcirculatory Perfusion along Meridians of the Back in a Stable Condition or After Moxibustion Treatment</strong></p><p style="text-align: justify;">Abstract:Objectives: This study investigated blood perfusion coherence characteristics along the meridain of the back in healthy volunteers with or without moxibustion, in order to provide evidences to elaborate biological characteristics of the acupoints and meridains on back via laser speckle system. Methods A total of 19 healthy subjects were recruited for this study. The PeriCam Perfusion Speckle Imager System was used to investigate skin microcirculation perfusion of the Du Meridain and the first line of Bladder Meridian simultaneously before/during/after moxibustion (5-30min). Coherence analysis between these meridians were conducted by linear statistics. Results The perfusion unit of Du Meridian was higher than Bladder Meridian on the back of the healthy subjects (p&lt;0.05). Meanwhile, there were no significant difference between Bladder Meridians on both side; After moxibusion treatment, the perfusion units of acupoints on both of the Du Meridian and first line of Bladder Meridian has increased significantly compared to the non-meridian points in back. It may indicate the meridians have heat conductivity.Conclusions Microcirculatary perfusion on the merdians has difference compared with other area.
